An individualized plan of care to meet the older adult's needs:

WHAT DOES IT DO?

Enables older people to maintain as much independence as possible:

  • Reduces loneliness and isolation

  • Restores and rehabilitates to the greatest extent possible

  • Prevents premature and inappropriate institutionalization

  • Preserves dignity and enhances the older person's quality of life

A GOOD CHOICE FOR:

  • Older people who need care yet wish to remain at home

  • Families who work and cannot leave their older relative at home alone during the day

  • Caregivers who need respite from the constant care of a loved one

Older adults age 60 and over who reside in Fairfax County, Falls Church, and Fairfax City

HOURS

Monday through Friday, 7 a.m.- 5:30 p.m.

COST

Daily fee based on older adult's ability to pay
